chore: remove weird syntax sugar from macro command
Before this commit,
```
macro term x:term : term => `($x)
```
would generate the notation
```
syntax "term" term : term
```

fix: throwAppTypeMismatch should be polymorphic
We use it from `TermElabM` and `MetaM`, and they have different
`TermElabM` implementations.
